- Advance Directives - This is a great resource on the information your closest family and friends need to know about your wishes.
- Conversation Project, The - How to plan for conversations about your health care wishes, including how to sort out for yourself what your wishes are.
- Fall Prevention - This article by Jane Brody has great tips.
- Health Care Proxy - Here is the NYS Health Care Proxy form, and lots of information to help you understand how to choose a health care proxy, why it is important, and what that person needs to know about your wishes (see Advance Directives and The Conversation Project above).
- Guide to Orchestrating Your Life as You Age, A - This guide helps you understand how to complete tasks and set up documents so that you can feel more confident
- Living Will Registration - Complete this document and send it in. Assuring Your Wishes will allow you and your doctors access your living will from any internet connected computer.
