Medical Note-Taking

Medical Note-Takers are recorders of the communication between the medical provider and the CAN member. A medical note-taker provides these benefits:
  • Acts as another pair of ears at the medical appointment. Listens and observes.
  • Focuses on communication between the medical provider and the CAN member
  • Asks clarifying questions when necessary to ensure that the medical provider's terminology is understandable to the patient
  • Empowers members to speak for themselves
  • Documents the content of the medical visit for the CAN member and for one of her/his family member (if needed). Note that after providing this documentation, the CAN note-taker does not keep a copy.
  • Always maintains confidentiality for the CAN member’s privacy
Medical Note-Takers are NOT Patient Advocates and DO NOT GIVE ADVICE or RECOMMENDATIONS to the CAN member.

CAN Medical Note-Takers completed a training program that was developed by Northwest Neighbors Village (NNV) in Washington, DC in partnership with a local senior service agency in Washington, Iona Senior Services. CAN purchased the rights to this training program in 2023.
